General Waiver: Duke and Dixie's Dog Grooming employs extensive safety and sanitary precautions to ensure your pets safety and health. Despite reasonable and prudent efforts, some pets may experience common side effects of the grooming process, including but not limited to: clipper burn, exposure of the quick, reactions to flea treatments and de-shedding treatments, hair splinters, and sharp nails (after clipping). By signing this agreement, you (the client) agree to relieve Duke and Dixie's Dog Grooming, and it's agents from any and all liabilities, and/or costs associated with any veterinary care, symptoms or discomfort of your pet related to or presenting after services rendered by Duke and Dixie's Dog Grooming. The pursuit of any veterinary, behavioral, or rehabilitational services, and the payment there of, is the sole responsibility of the pet owner. Additionally, you agree to relieve Duke and Dixie's Dog Grooming and its agents from any and all liability associated with damages to person, pet, or property by or relating to services rendered or service equipment used by Duke and Dixie's Dog Grooming and it's agents.  

Vaccinations:  It is a requirement that ALL dogs be vaccinated for both Rabies and Bordetella (Kennel Cough) at time of services.  It is highly recommended that vaccinations be given a minimum of 10 days prior to appointment in order to be fully protected.  We will keep a record of your vaccinations on file for future appointments.  

Pick up-Drop Off: The pick-up time given is only an estimate, and we will call the owner to give actual pick up time. Each pet is different and will require different lengths of time for grooming. Unless owner has paid for express service, there will not be a set time for services being rendered.   

Matting Agreement: Unless a dematting service has been approved by owner, groomers will be removing matting with shears or clippers from the body of the dog. Owner has been made aware of the possibility of hematomas that can occur after heavy matting has been removed. The Dematting charge is $5 PER 10 MINUTES of dematting services. If owner agrees to dematting service, they understand and agree to all charges associated with it by signing this document.   

Hazard Handling: In case of severe anxiety, aggression or fear, the owner understands and agrees by signing that services may not be fully completed and/or a hazard handling fee will be added. Groomers retain the right to refuse service for a pet that is creating an unsafe environment for the groomer or other pets in the salon.
